To place an order, you will need to print out the order form, fill it out and decide which delivery option you would like (listed below). Send your order along with a check made out to "BMDCO" to Dawn (her address is on form).

HOW TO ORDER:
FILLING OUT THE ORDER FORM
Step 1: Print out this ORDER FORM
Step 2: Select the merchandise you want along with the type of logo (large silk screen or smaller embroidery) you want and fill in the appropriate areas on order form.
Step 3: Select delivery option from next box
Step 4: Write check for merchandise and shipping costs to "BMDCO" and mail with order form to Dawn Weber (address below).
DELIVERY OPTIONS (PICK-UP VS. USPS)
OPTION 1 - PICK-UP (NO SHIPPING CHARGES)
If you wish to pick up your order from Dawn, just fill out the order form and write out a check made to "BMDCO" for the amount of the merchandise. Be sure to note on the order form that you will pick up your order. Send your order form and check to address below.
OPTION 2 - SHIPPING VIA USPS (CALL DAWN WEBER FOR COST PRIOR TO MAILING YOUR ORDER)
If you want your order shipped to you than before writing out your check (to the "BMDCO", by the way) contact Dawn Weber to find out what your shipping charges will be. You will need to tell her which merchandise you will be ordering so she can figure out the cost. HOWEVER, you will still need to send her a completed order form with your check.
